I can see why Ryan Graves is spearheading a reporting process for Pilots. The communication between the pilot and air traffic control shows how these occurrences are being observed yet no process is in place (at the time). You can hear the hesitation from the pilot to make a report, this is exactly what Ryan is trying to address!
